Non-Surgical Body Contouring: Your Options Explained

Non-surgical body contouring has become one of the fastest-growing areas in aesthetic medicine. For patients who exercise regularly and maintain a healthy diet but still struggle with stubborn pockets of fat, loose skin, or areas that simply will not respond to lifestyle changes alone, these treatments offer a way to refine body shape without surgery, general anaesthesia, or extended downtime. In our Maidenhead clinic, we see a growing number of patients — both men and women — who are interested in understanding what these technologies can realistically achieve.

It is important to be clear from the outset: non-surgical body contouring is not a weight-loss solution. These treatments are designed to target localised areas of fat, improve muscle tone, or tighten skin in specific regions. They work best for patients who are already close to their target weight but have areas that are resistant to diet and exercise. If significant weight loss is the primary goal, a conversation with your GP about nutrition, exercise, and potentially medical weight management is a more appropriate starting point.

This guide provides a straightforward overview of the main non-surgical body contouring technologies available in the UK, explains who is most likely to benefit, and sets honest expectations about results. Understanding your options

Non-surgical body contouring broadly falls into three categories, each addressing a different concern. Understanding the distinction between them is important because no single technology does everything — and the right treatment for you depends on what you are trying to achieve.

Fat reduction: cryolipolysis (CoolSculpting)

CoolSculpting uses a process called cryolipolysis to permanently destroy fat cells by controlled cooling. An applicator is placed on the treatment area, drawing the tissue into a cup and lowering the temperature to a point where fat cells undergo apoptosis (programmed cell death) while leaving surrounding skin, nerves, and muscle tissue unharmed. Over the following weeks, the body naturally processes and eliminates the dead fat cells through the lymphatic system.

The science behind cryolipolysis is well-established. Fat cells are more susceptible to cold than other tissue types, and clinical studies have demonstrated a reduction of approximately 20–25% of fat in the treated area per session. The treatment is FDA-cleared and has been the subject of extensive peer-reviewed research. It is important to understand that this is a reduction in a specific area — it is not a dramatic transformation, but rather a refinement.

Muscle building and fat reduction: HIFEM + RF (Emsculpt Neo)

Emsculpt Neo combines two technologies: high-intensity focused electromagnetic energy (HIFEM) and radiofrequency (RF). The HIFEM component induces supramaximal muscle contractions — the kind of deep, intense contractions that are impossible to achieve through voluntary exercise. A single 30-minute session triggers approximately 20,000 muscle contractions. Simultaneously, the RF energy heats subcutaneous fat, damaging fat cells and contributing to fat reduction.

Clinical data for Emsculpt Neo shows an average 25% increase in muscle mass and 30% reduction in subcutaneous fat in the treated area. This dual action makes it particularly useful for patients who want both improved muscle definition and fat reduction — for example, someone who wants a more toned abdomen or firmer buttocks.

Skin tightening: radiofrequency (Exilis)

Exilis uses focused radiofrequency energy to heat the deeper layers of the skin, stimulating collagen production and improving skin laxity. It is particularly effective for patients whose primary concern is skin looseness or crepiness rather than excess fat — for example, mild skin laxity on the arms, inner thighs, or abdomen following weight loss or ageing.

Radiofrequency skin tightening works gradually. The heating triggers a wound-healing response in the dermis, leading to new collagen and elastin formation over several weeks. Results continue to improve for two to three months after a course of treatment. While the degree of tightening is modest compared to surgery, for patients with mild to moderate laxity it can produce a noticeable improvement.

Who is a good candidate?

The best results from non-surgical body contouring are achieved by patients who meet certain criteria. During your consultation at our Berkshire clinic, we assess each patient individually, but as a general guide, ideal candidates share the following characteristics:

  • Within approximately one to two stone of their target weight, with a BMI generally under 30
  • Have identifiable areas of stubborn fat that have not responded to consistent exercise and a balanced diet
  • Understand that these treatments produce gradual, moderate improvements — not dramatic transformations
  • Are in good general health without contraindications such as cryoglobulinaemia (for CoolSculpting) or metal implants in the treatment area (for Emsculpt Neo)
  • Are not currently pregnant or breastfeeding

Equally important is understanding who is not a suitable candidate. If a patient has a significant amount of weight to lose, non-surgical body contouring will not produce meaningful results. These technologies reduce localised fat by a percentage — if the fat layer is too thick, a 20–25% reduction may not be visually noticeable. In these cases, we are honest with patients and recommend they focus on weight management first. We would rather decline a treatment than deliver a disappointing result.

Patients with very loose, hanging skin (such as after major weight loss) are also unlikely to see sufficient improvement from non-surgical skin tightening alone. In these situations, surgical options such as abdominoplasty may be more appropriate, and we can discuss referral pathways during your consultation.

Treatment areas and what each addresses

Different body areas respond to different technologies. Part of the clinical assessment process involves identifying which treatment — or combination of treatments — will produce the best outcome for each patient’s specific concerns.

  • Abdomen:One of the most commonly treated areas. CoolSculpting is effective for reducing subcutaneous belly fat. Emsculpt Neo is particularly valuable here, as it simultaneously reduces fat and strengthens the abdominal muscles, producing a firmer, more toned appearance. Exilis can address mild skin laxity on the lower abdomen.
  • Flanks (love handles):CoolSculpting is especially well-suited to the flanks, where pinchable fat can be drawn into the applicator effectively. This is one of the areas where patients tend to see the most satisfying results from cryolipolysis.
  • Thighs:Both the inner and outer thighs can be treated with CoolSculpting for fat reduction. Exilis is often used on the inner thighs for skin tightening where laxity is the primary concern. Emsculpt Neo is not typically used on the thighs.
  • Arms:Upper arm fat and skin laxity are common concerns, particularly in women. CoolSculpting can reduce fat in this area, while Exilis addresses skin looseness. The combination of both can produce a more defined upper arm contour.
  • Buttocks:Emsculpt Neo is the primary treatment for the buttocks, where it builds and firms the gluteal muscles. This is not about adding volume in the way that surgical augmentation does, but rather about improving tone, lift, and shape through muscle strengthening.

What to expect: sessions, results timeline, and maintenance

One of the most common questions patients ask is how quickly they will see results and how many sessions they will need. The answer depends on the treatment:

CoolSculpting

Most patients require one to two treatment sessions per area. Each session takes 35 to 60 minutes depending on the applicator used and the area being treated. Results are not immediate — the body takes time to process and eliminate the destroyed fat cells. Most patients begin to notice visible changes at around six to eight weeks, with optimal results typically seen at 12 weeks. Because the fat cells are permanently destroyed, results are long-lasting provided the patient maintains a stable weight. Significant weight gain after treatment can cause remaining fat cells to enlarge, diminishing the effect.

Emsculpt Neo

The standard protocol is four sessions over approximately two weeks, with each session lasting 30 minutes. Some patients feel a difference in muscle tone within a few days of starting the course, but the full effect — both in terms of muscle building and fat reduction — typically develops over two to three months as the body continues to build muscle fibres and eliminate damaged fat cells. Maintenance sessions every three to six months are recommended to sustain results, similar to how you would maintain fitness gains through ongoing exercise.

Exilis skin tightening

A course of four to six sessions is typical, spaced one to two weeks apart. Each session takes 20 to 30 minutes. Collagen remodelling is a gradual process, so results develop progressively over two to three months after completing the course. Annual maintenance sessions can help preserve the improvement, as natural collagen loss continues with ageing.

Across all these treatments, lifestyle factors play a significant role in both the quality and longevity of results. Regular exercise, a balanced diet, adequate hydration, and maintaining a stable weight all contribute to better outcomes. We discuss these factors during every consultation because we believe in treating the whole patient, not just the treatment area.

Risks, side effects, and honest limitations

All medical and aesthetic treatments carry some degree of risk, and non-surgical body contouring is no exception. While these treatments are generally well-tolerated, patients should be aware of the potential side effects and limitations before proceeding.

CoolSculpting side effects

Common side effects include temporary redness, swelling, bruising, and numbness in the treated area. The numbness can persist for several weeks in some patients but resolves on its own. Mild cramping and tingling are also reported. A rare but important complication is paradoxical adipose hyperplasia (PAH), where the treated area develops an enlargement of fat rather than a reduction. PAH occurs in approximately 0.02–0.05% of treatments and is more common in men. While not medically dangerous, PAH does not resolve on its own and typically requires liposuction to correct. We discuss this risk with every patient during their consultation.

Emsculpt Neo side effects

The most common side effect is muscle soreness, similar to what you would experience after an intense workout. This typically resolves within 24 to 48 hours. Mild redness or warmth in the treatment area from the radiofrequency component is normal and fades quickly. Serious side effects are extremely rare. Patients with cardiac pacemakers, metal implants in the treatment area, or certain medical conditions cannot be treated.

Honest limitations

Perhaps the most important thing we can tell patients is what these treatments cannot do. Non-surgical body contouring will not replicate the results of surgical procedures such as liposuction or abdominoplasty. The changes are real but moderate — patients should expect a refinement of their body shape, not a transformation. Photographs and measurements show the difference, but the improvement is typically subtle enough that friends and family may not notice unless told.

We also advise caution regarding home devices marketed as alternatives to professional body contouring treatments. At-home fat freezing belts, EMS devices, and RF gadgets available online are not equivalent to clinic-grade equipment. Professional devices such as CoolSculpting and Emsculpt Neo are precisely calibrated medical devices that have undergone rigorous clinical testing. Home devices lack the power, precision, and safety monitoring to deliver comparable results, and using them incorrectly can cause burns or skin damage. If a treatment seems too cheap or too convenient to be true, it almost certainly is.

For many patients, the best results come from a combination approach.
